My technique for kitty-pilling is to kneel on the floor, put the cat between my knees facing forward, squeese my knees together enough to partially hold the cat, then use one hand to force his jaws open while also appling a bit of pressure to keep it from escaping. The other hand jams the pill in. Once it is in, I use one hand to hold his mouth shut and the other to stroke his throat until he swallows. With practice, this works fairly well even with wildly struggling cats. It doesn't work on small kittens though, since they're not easily gripped between my knees.
